深層腦部刺激 巴森症走路不僵硬

73歲施先生6年前因顫抖和行動緩慢，診斷是巴金森氏症，後來出現肢體僵硬，走路走到一半突然定住的現象，服用很多抗巴金森藥物，效果有限。經台南市立安南醫院神經內外科合作施行深層腦部刺激手術（ＤＢＳ），術後恢復良好，肢體僵硬已有改善。

腦神經內科李建欣醫師表示巴金森氏症分為五期，到了末期，患者不但無法站立，嚴重者甚至連翻身都有困難。臨床上主要以藥物治療和緩解相關症狀，但長期服藥後，易產生異動症及開關現象，導致病患的日常生活起居受到極大的困擾。ＤＢＳ是近10年來，治療巴金森氏症醫學的新療法，輔佐藥物雙管齊下，可以有效地改善病情。

李建欣指出，患者如果有以下狀況，則不建議接受ＤＢＳ：1、對左多巴藥物治療反應不良的非典型巴金森病患者。2、疑有精神異常疾患或嚴重的憂鬱症。3、合併有中度以上失智症。4、合併有癌症、肝腎功能不佳或其他出血性、全身性疾病。

李醫師說，健保署自104年起將「深層腦部刺激器」納入健保給付，此種醫材可產生電流來調節病患腦內不正常的神經迴路，改善病患過多或異常的動作（異動症），且有效減少病人未來幾年的藥物用量、看病次數、併發症等。
